The first step is for us to speak directly. This will give you an opportunity to evaluate whether you want to choose me to help with your problems or concerns. I can also answer questions about scheduling and payment options.

Prior to your first visit, please download and print the following forms, and then:

• Carefully review the Privacy Policy. If you have any questions, I will be pleased to assist you.
• Sign and date the Receipt of Privacy Policy. This form is an acknowledgement of having received a written copy of the Privacy Policy.
• Bring your signed receipt with you when you next visit my office.
